The participant hereby declare warrants and represents to the organizer that are 18 year old & above.
(Age below years old is required to complete the Parental consent section of the online registration form)
As a matter of caution, participants are ought to:
1) Sought medical examination ensured that the participant is certified to be medically fit to participate in this event.
2) Wherever applicable, have ought to ensure that the participant's insurance policy is not void or voidable as a result the participation of this event.
3) Have ensure that participant sporting equipment and safety gear have recently check by skilled bicycle mechanic in the event and services to be fit for use in this event.
4) have take cognizance of the fact that the trail designated for this event is extremenly challenging and hence required much endurance technical skill to surmount.
Online registration process:
1. Fill in the registration form by clicking the link below.
2. After you have submit your registration, you will be receiving an email notifying to confirm your registration submission and your registration number. Please allows a few hours to 1 day to receive the email.
3. Make payment of RM60 to "MAJLIS SUKAN DAN KEBAJIKAN JABATAN KESIHATAN NEGERI PERLIS" at CIMB Bank, account number "7045711842".
4. If you do your payment through your online banking account, please enter either your name or registration number in the "Recipient Reference" field for faster verification.
5. Send a scan copy of the payment transaction note to "kayuhanmindasihat@gmail.com".
6. Your registration will only valid after the payment is verified, and an email will be sent to you to confirm your registration. You can also check your registration status by viewing the participant list (please allow 1 week to update the list).
7. If payment made through 'bank in' please scan or email your bank slip together with entry form to kayuhanmindasihat@gmail.com
